What is the Balke treadmill test?
Balke and Ware (1959) holds the treadmill at a walking speed and raises only the gradient, one step at a time, until you cannot continue. Because the belt never speeds up past a walk, it can be given to people a running test would rule out — cardiac rehabilitation patients, older adults, anyone being screened rather than trained. The score is simply how long you stayed on.
How this calculator works
Two equations, one per protocol variant. Men walk at 3.3 mph with the grade going to 2% in the second minute and up one point every minute after, giving VO₂ max = 1.444 × minutes + 14.99. Women walk at 3.0 mph with the grade rising 2.5 points every three minutes, giving VO₂ max = 1.38 × minutes + 5.22. Each line is fitted on its own variant, so a time from one must never be read on the other's equation.
Running the test properly
Set the belt to the right speed for the variant and hold it there for the whole test — the only thing that changes is the incline. Do not hold the handrails: leaning on them offloads a real fraction of your bodyweight and inflates the time badly, which is the single commonest way this test gets a wrong answer. Stop at genuine exhaustion, and have someone supervising if there is any cardiac history at all.
How accurate is it?
Submaximal walking protocols estimate rather than measure, and the standard error is a few ml/kg/min either way. The estimate assumes you walked to the point where you truly could not carry on; stopping early because the incline felt unpleasant reads as low fitness. It also has a ceiling — a trained runner can walk up a steep grade for a very long time without reaching their limit, and is better served by the Bruce protocol or a running test.
